Carol

Carol was previously diagnosed with a schizophrenic disorder which impacts her current functioning in that she is unable to work; however, Carol manages her disorder with the help of anti-psychotic medication which she takes regularly. Her short and long-term memory are "good," but she has "difficulty paying attention" and her processing of information is "somewhat slow." Therefore, Carol's ratings on the Thought Process and Cognitive Performance domains fall within the 3 - 4 and 3 -5 range, respectively.

Carol presents with symptoms of depression. She is "crying a lot," has not been sleeping much, and has difficulty concentrating on routing chores. The death of her father is a recent stressor. Valid ratings for the Depression domain are between 3 and 5. Carol describes a broad network of family and friends with whom she keeps in touch, and Carol has the support of her husband with whom she has been married for 22 years. Carol's son keeps regular contact. Interpersonal Relationships are generally strong, but Carol has not been visiting with friends as much as usual since the death of her father. Valid ratings for the Interpersonal Relationships are between 2 and 4.

In addition, Carol reports feeling "agitated and anxious... worrying about her mother." Carol's anxiety is related to recent stressors. Valid ratings for the Anxiety domain are between 2 and 4. She continues to be in good health but with a head cold currently. Valid ratings for the Medical/Physical domain are between 2 and 3. Carol does prepare meals for herself and her husband but has not been eating as much lately. Valid ratings for the ADL Functioning and Ability to Care for Self domains are 1 - 2 and 1 - 3, respectively.

According to the vignette, "Carol says she does not drink alcohol or use illegal drugs and has never had a problem with the law." Carol has no issues with respect to the Socio-Legal and Substance Use domains. Carol has a prior suicide attempt under conditions that do not impact her current functioning, so issues with respect to the Danger to Self domain are also minimal.
